RA Signature Form

 To record that your staff and students have read and understood the RAs listed in the relevant PSSW, we have an electronic method of signing risk assessments. They should add the reference number and title of each RA into the RA signatory form and sign it.

You, as their PI, then countersign acknowledging that the RAs are appropriate for the work.

This document should be updated as they start new techniques, at least annually.
