It is important to report near misses as it indicates that the RA in place for a particular procedure needs to be updated to prevent an accident. After you have filled out the form, you will receive an email from the H&S department asking you to review your RA for the activity you were doing to see if additional controls are required. If you need to amend the RA please ask Sarah or Cathy for the appropriate form, review and amend the RA send them back to be reviewed and uploaded to the RA folder.
